How much D5% do you use to dilute 1 gram of Vancomycin?

I am at home receiving 1 gram of vancomycin every 12 hours. The problem is I have 2 different nurses and the argument regarding how much D5% to dilute it in are ongoing. I have spent an afternoon on the net and can not find the answer anywhere. Please help. Thank you in advance for your help in this matter.

According to http://www.drugs.com/pro/vancomycin.html concentration should be no more than 5 mg/mL. For example 1gm of Vancomycin should be mixed in at least 200 cc of fluid. It should always be run over 1 to 2 hours.
